What additional information is required along with the questionnaire?
Additional information required includes P&ID, original datasheet and fluid properties, operating data - temperature, pressure and flowrate (minute-data in an excel file preferred), pictures of the leakage or damages, and operational history - shutdown, ervice, cleaning, inspection, etc.
